This DVD was included in the Biohazard 5th Anniversary Special Package ~Nightmare Returns~ (SLPM 86770) box set, and also given as a bonus for pre-ordering Biohazard Code: Veronica Complete (SLPM 65022~3).

The disc contains the title video, a Biohazard Code: Veronica Complete trailer, and Directors' Hazard, a conversation among the developers. It came with two sheets.
