Following the events of Hitman 2, Agent 47 and his friend Lucas Grey begin their hunt for the leaders of Providence: the Constant, Arthur Edwards, and his three "Partners", Carl Ingram, Marcus Stuyvesant, and Alexa Carlisle. They begin by infiltrating Burj Al-Ghazali, the world's tallest building, in Dubai during its inauguration, where 47 eliminates both Ingram and Stuyvesant, but Carlisle leaves before she can be killed. After the hit, Edwards locates and contacts 47's handler Diana Burnwood, who realizes that the Partners' deaths and Grey's contracts from Hitman were all orchestrated by Edwards himself.
Agent 47 travels to Carlisle's mansion in England, where the Carlisle, Ingram, and Stuyvesant families were brought together for the first time to form Providence, to assassinate Carlisle and retrieve a case file she keeps on Edwards. After the mission, private security firm CICADA, who were hired by Edwards to eliminate 47 and Grey, locate the latter, and he warns 47 that Diana can no longer be trusted and that he needs to find his partner, hacktivist Olivia Hall, before committing suicide. 47 disguises himself as a CICADA security guard to contact Hall and tell her that Grey is dead and that she needs to flee to Berlin. Meanwhile, Edwards attempts to win Diana over by giving her a case file that reveals 47 as her parents' killer, and offering to make her the next Constant.
47 meets with Hall in a Berlin club, but they are forced to flee when agents from 47's former employers, the International Contract Agency (ICA), arrive looking for them. After eliminating all the ICA operatives, 47 meets with Hall again in an abandoned diner, where she informs him that the data they have on Edwards is useless. With no other solution, 47 infiltrates the ICA's main data storage facility in Chongqing, China, and eliminates its two overseers, "Hush" and Imogen Royce, to weaken the security protocol, allowing him to wipe all traces of his and Diana's existence and leak all the stored data to public websites. Meanwhile, Diana seemingly joins Edwards and betrays 47, revealing his only weakness: herself.
Diana sets up a false mission for 47 in Mendoza, Argentina: to assassinate Providence members Don Yates and Tamara Vidal. When he realizes this and comes to confront her, Diana uses an Ether neurotoxin to knock him out. Before 47 passes out, Diana tells him that she knows what he did to her parents, and that Providence used him, but she did too, so she is freeing him. After recalling past contracts as the room slowly fills with blood, 47 wakes up in a bed, face to face with Edwards, who tells him that he will be making him his personal tool, and that he lied to Diana about killing 47 so that she would deliver the assassin to him. As Edwards prepares to inject him with an amnestic serum, 47 passes out again and sees Grey, who encourages him to fight the serum's effects, and tells him that Diana never wanted him dead, but rather found a way to turn Edwards' power against himself. 47 then hears Diana's voice telling him to embrace the past, causing him to relive killing her parents.
47 wakes up in Edwards' lab at the old asylum in Romania where he was created, and boards a moving Providence train to kill Edwards, before fleeing into the forests of the Carpathian Mountains. Later, with Providence finally dismantled, Diana watches the news in her cottage, revealing that many business CEOs (who, unbeknownst to the public, are former Providence members) have resigned from their positions in a bizarre pattern. She then takes the case file of her parents' death and throws it into the fireplace, along with an origami crane resembling the Providence badge.
In the epilogue, one year later, 47 is walking through the forests while telling Diana on the phone that he is returning to perform hits willingly, as it is his life and their check on elites' power will always be required. Diana tells him, "No one is untouchable," causing 47 to briefly smile before replying, "It's good to be back."
