A car-shaped character circles in one of four lanes around the screen collecting dots while computer car(s) attempt to crash head-on into the player. There are only four locations on the screen to change lanes. Collecting all the dots advances the player to next level with either more or faster opponent cars.
This game was manufactured in four cabinet styles: upright arcade, cocktail, stand around and mini. Besides being a stand-alone game, Head On 2 was also released as part of a dual game cabinet, paired with either Invinco! or Deep Scan. The marquee was divided in half with one name on the left and the other on the right. There were also two sets of controls; one for each game.
